Registered Nurse (RN) Diabetes Educator provides care in an
atmosphere sensitive to each person's physical, emotional, social,
and spiritual needs in accordance with the mission of Bon Secours
Mercy Health and within the scope of the Ohio Nursing Practice Act.
A RN Diabetic Educator educates and supports people affected by
diabetes to understand and manage thecondition. A RN Diabetic
Educator promotes self-management to achieve individualized
behavioral and treatment goals that optimize health outcomes. The
RN Diabetic Educator assesses, develops, implements and monitors
the nursing plan of care and coordinates this with all other
applicable providers. This requires awareness of all diagnoses and
seeking input when needed. The RN Diabetic Educator assists
patients to achieve and maintain quality, cost-effective goals.
Also, the RN Diabetic Educator assumes a personal responsibility
for professional growth. The RN Diabetic Educator will incorporate

daily practices. The RN Diabetic Educator, functioning in the
spirit of teamwork, is accountable for the delivery of care using
evidence based standards, policies, procedures, and guidelines of
the organization.Essential Duties and Responsibilities:1. Gives
direct nursing care which is commensurate with the education,
knowledge, skills, and abilities of the registered nurse.2. Obtains
and follows through with physician referrals.3. Assumes
responsibility of collecting appropriate data necessary to
familiarize self with patient's medical history and diagnostic
testresult.4. Assesses objective clinical endpoints that relate to
disease state control.5. The frequency of a nursing assessment
shall be based upon the RN's judgment and the patient's status.6.
Assess subjective data.7. Identifies needs in areas such as
socioeconomic, family resource, and knowledge.8. Involve, as
appropriate, the patient, their family, significant others, or
other members of the healthcare team in the evaluation and plan.9.
Creates effective teaching tools to present concepts to the
patient.10. Analyze assessment data and establish, accept or modify
the problem list.11. Accurately reports variances in patient
physical and/or emotional condition to the appropriate provider.
Sends out updates to referring physician on a routine basis.12.
Evaluates outcomes and initiates any modifications in plans
necessitated by the patient's condition with physician approval, as
appropriate.13. Assumes responsibility for carrying out orders.
Assumes responsibility of assuring patient/family understanding of
all orders.14. Identifies and/or coordinates follow-up visits or
phone calls.15. Evaluate, document and communicate a patient's
response to interventions.16. Identifies and coordinates
appropriate team members for development of a multi-disciplinary
individualized plan of care. This may include pharmacy, social
services, dietary, rehab, home health, hospice and any other
treating providers.17. Assumes responsibility for ongoing
assessment of patient needs through frequent patient contacts and
communication with all providers.18. Administers prescribed
medication according to MHP and Hospital protocol and policies.19.
Acts as a liaison between MHP and community health resources.20.
Increases disease state awareness to the community through
presentations at community groups, schools, and health fairs.21.
Establishes and maintains charges for clinic interventions.22.
Participates in implementing Patient Satisfaction Survey and any
other appropriate PI projects.23. Keeps staff and patients current
on disease state issues being discussed in the media.24. Maintains
awareness of patient safety, privacy, and confidentiality.25. Is
responsible for performing all other duties as assigned.Performs

this job successfully, an individual must be able to perform each

ACLS required for any clinics dealing with cardiology.Education and
/or ExperienceEducation: BachelorsYears of Experience: 1-3Must be a
graduate of a college or university with an accredited School of
Nursing, a Bachelor's degree preferred.Two years experience in a
critical care setting with experience in patient education and
teaching required. Must obtain a certification as a Certified
Diabetes Educator within 2 years of employment. Although prior
employment in an ambulatory setting is preferred, other skills and
experience will be weighted accordingly.Communication/Interpersonal
SkillsThis employee must be able to communicate with coworkers,
patients, physician offices, and other departments at the Medical
Center. Accuracy and clarity are required when interacting with
elderly and individuals who demonstrate medical/cognitive
dysfunctions. Must be able to perform multiple tasks simultaneously
and accurately. The employee needs to have the ability toconvey the
importance of appointments and communicate the reason for changes
to patients. Changing lifestyle habits requires the ability to
communicate at multiple levels and in creative manner. Able to take
complex topics and bring them to a level of simplicity which can be
understood by the patient. Communicates on a 1:1 basis, group
setting and with family members. Provides healthcare in a
non-judgmental manner, recognizing personal and cultural diversity.

organize work, set priorities, and inform Manager when exceptions
need to be made. Must be able to handle difficult patient
situations. Ability to solve practical problems in situations where
