Job Description:
The RN Care Manager role is a full-time, driving position
serving Wooster and surrounding areas, requiring collaboration with
colleagues and a passion for hospice care.
Responsibilities include assessing, planning, implementing, and
evaluating patient care while interacting with patients, families,
and team members.
Qualifications: graduate of an accredited nursing school, RN
license in Ohio, CPR certification, computer skills, and a valid
driver--------s license with a safe driving record.
Preferred: two years acute care experience, hospice and palliative
care certification, and the ability to pass background checks and
drug screens.
