in post-acute care facilities Observing the conditions of residents
and notifying the nursing team of updates Monitoring and reporting
on residents' vital signs Ensuring a clean and safe environment by
performing tasks such as sanitizing surfaces and changing linens
Assisting residents with personal hygiene, feeding, grooming, and
other activities of daily living (ADLs) Repositioning residents and
transporting them around the facility as needed Liaising between
staff, residents, and family members to support ongoing
communication and transparency Engaging with residents on a regular
basis and responding to their needs Being accountable to follow and
adhere to care plans for residents, and all applicable laws,
regulations, and policies Qualifications and Skills Current Ohio
